Four care pathways, each with its own funding route

Home health care is not one service. Which pathway a patient falls into determines who authorises the care, how soon it can start, and which clinicians we send.

Pathway 01 · insurance or private

Adults & seniors

Ongoing care at home for adults living with chronic illness, reduced mobility or the effects of ageing. A registered nurse assesses the situation, builds the plan of care and supervises the team that delivers it.

  • Skilled nursing on an hourly or shift basis
  • Home health aides for personal care and daily living
  • Medication management and wound care
  • Therapy at home — physical, occupational, speech
Pathway 02 · full-scope Medi-Cal

Children & young adults — EPSDT

EPSDT supplemental services for patients 21 years and younger who hold full-scope Medi-Cal. We provide hourly or shift nursing in the family home, together with support from therapists and medical social workers.

Paediatric home nursing asks more of a team than adult care does: continuity matters, families need to trust the same faces, and school and therapy schedules have to fit around the care. We staff these cases accordingly.

Pathway 03 · physician referral

Recovery after hospital

The weeks after a discharge decide how well someone recovers. We pick up patients leaving hospital or a skilled nursing facility and carry the plan of care into the home — wound care, medication supervision, IV therapy, and rehabilitation after stroke or surgery.

  • Referrals accepted 24/7, including at weekends
  • Initial nursing assessment in the home
  • Progress reported back to the ordering physician
Pathway 04 · hospice support

Respite & end-of-life care

We provide care for clients in the terminal stage of their illness — comfort-focused, unhurried, and centred on dignity. We also provide respite so that families who have been caring alone can rest.
