How it works

Three steps, and the paperwork is ours

Send the referral

Phone, secure form or fax: patient details, diagnosis, insurance and the ordering physician. Partial information is fine — we will chase the rest.

We verify and authorise

We confirm coverage, obtain the signed physician's orders and follow the authorisation through, so your office does not carry it.

Care starts — you get confirmation

A registered nurse completes the initial assessment in the home, your office receives confirmation, and the ordering physician receives progress reports.

Before you send

How fast can care start?Where staffing allows we accept same-day discharges. The initial assessment is scheduled as soon as the physician's orders and coverage are confirmed — call the intake line and we will tell you on the call whether we can take the patient.
Which areas do you cover?Stanton and the surrounding Orange County cities — see coverage. Ask about anything outside that; we will tell you honestly whether we can staff it.
Do you take Medi-Cal?Yes — including EPSDT supplemental services for patients 21 and under with full-scope Medi-Cal.
Which disciplines are in-house?RN, LVN, CHHA, physical, occupational and speech therapy, and medical social work.
Do families need to speak English?No. We provide various language interpreters for patients and families at no additional cost.
Will I hear back?Yes. Your office receives confirmation of acceptance, and the ordering physician receives progress reports.
